If you are a resident of Mercer, Paulding, or Van Wert Counties, you may contact any of our two primary Providers with three locations:
– Mercer County – Foundations Behavioral Health Services 419-584-1000
– Paulding County – Westwood Behavioral Health North 419-399-3636
– Van Wert County – Westwood Behavioral Health Center 419-238-3434
These Providers have Open Access hours and you may be able to be seen the day you call. You will receive a clinical assessment provided by a licensed clinician and then be scheduled to receive services in the least restrictive environment. The local Providers offer outpatient services at their respective locations and serve as a linkage hub to obtain admission at inpatient hospitals or specialty residential providers if such services are considered to be clinically appropriate.
Costs are paid by a combination of Medicaid, Medicare, private health insurance plans, and local subsidy provided by the Tri County ADAMHS Board. Clients may be responsible for co-payments based on individual income level. No one is turned away due to the lack of ability to provide payment.
